Where the money goes

A manufacturer copay card can bring a commercial-insurance copay down sharply. Not usable with Medicare or Medicaid.
If your plan denies it, a formulary exception backed by your doctor is often granted when the indication is heart failure or kidney disease rather than diabetes alone.
Medicare Part D now has an annual out-of-pocket cap, which changes the arithmetic on drugs at this price.
